Tonya Kreml is the founder of Transitioning Your Senior, LLC, launched in 2022 after more than 30 years of experience as a physical therapist assistant. Her passion lies in helping older adults safely age in place by offering personalized home safety assessments, caregiver education, and dementia-friendly modifications. Based in Central Ohio, Tonya also facilitates two Alzheimer’s support groups and partners with the local Alzheimer’s Association chapter to promote fall risk awareness. Through her work, she has helped hundreds of families understand evolving care needs and make informed decisions for their loved ones’ well-being.
