Observation Skills: Reporting and Documenting

Caregivers are often the first to notice small changes that may signal a change in a client’s health, behaviour, or well-being. **In British Columbia, clear observation, documentation, and timely reporting help the care team respond to changes and support safe, coordinated, person-centred care.** This course teaches caregivers how to observe objectively, distinguish between subjective and objective information, report concerns promptly and accurately, and document clearly for the care team. Through real-world examples, learners will understand how strong observation and documentation support effective client care and communication.
